29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. Employees working from a tubular welded frame scaffold are exposed to falls of approximately nineteen (19) feet six (6) inches to the ground due to the lack of guardrails being installed. This was most recently observed on March 19, 2019 at the jobsite located at 1340 W. Walton Street, Chicago, IL. 60642. Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including an explanation of how these steps protect its employees from fall hazards. Polo Masonry Builders, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1926.451(g)(1) which was contained in OSHA inspection number 1335739, citation number 2, item number 4 and was affirmed as a final order on February 14, 2019, with respect to a workplace located at 1747 W. Rosehill Drive, Chicago, IL 60660. Polo Masonry Builders, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1926.451(g)(1) which was contained in OSHA inspection number 1001826, citation number 2, item number 2 and was affirmed as a final order on April 28, 2015, with respect to a workplace located at 1000 W.
