29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: a. On or about March 29, 2019, at the above addressed jobsite, an employee was required to use fall protection equipment during roofing operations which was not inspected prior to use, thereby exposing the employee to fall hazards. Equipment being used had the following defects: Worn/burned/damaged life lines, lanyard (shock absorber) was damaged.
