29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval. This violation occurred on or about September 16, 2019 and at times, prior thereto, the employer allowed a load to be suspended from a rough terrain forklift tine. This exposed the employees to struck-by and caught-in hazards.
