29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: The employer is failing to protect employees from engulfment and struck-by hazards associated with working in a trench without adequate access and egress. The most recent example of this was on December 31, 2019 at or near 1505 Schoettler Road, Chesterfield, Missouri 63017, employees were replacing a 15 inch clay pipe in an excavation measuring approximately 20.5 feet deep X 45 feet long X 8 feet wide without a ladder, ramp or other safe means of egress.
