29 CFR 1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): At a worksite located at (40.53406 -95.45267) associated with the worksite of 30325 State Highway C, Westboro, MO: Employees were exposed to caught-under and/or crushing hazards when the jobsite was not inspected in its entirety. This was demonstrated on or about September 23, 2020 when an employee was operating a powered industrial truck (model # TL1255D, serial #ML 702775) in an area of the worksite that was not level and consisted of sloping terrain. The area had not been designated as a restricted operating area. The employer failed maintain frequent inspections of the job sit by reexamining their previous jobsite inspection to account for the sloped terrain and further failed to inspect that terrain and the surrounding area for potential hazards prior to the forklift being permitted to operate in the area.
