29 CFR 1915.159(a)(9): Anchorages were not capable of supporting at least 5,000 pounds (22.2 Kn) per employee attached: a) MHI, Normandy CG-60: On or about March 21, 2021, employees engaged in the removal and installation of Bolted Equipment Removal Plates attached their lanyard to a guardrail that was not capable of supporting at least 5000 pounds in that the guardrail was not anchored to the vessel structure thus exposed employee to an approximate 9 feet fall. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
