29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: On or about March 29, 2021, employees were exposed to a struck by hazard while performing logging sawmill operations at M&T Hardwoods, LTD. Employees were operating a Volvo 2200 model 7DZXL06-1063 Fork Truck to move and stack tree trunks in storage areas in the main yard and the employer did not ensure that the vehicle was taken out of operation when it was found to have a defective back up alarm.
