29 CFR 1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) Warehouse Area - On or about 04/29/2021, and at times prior, the employer exposed employees in the Tufting area to struck-by hazards in that inadequately trained employees were allowed to operate the powered industrial vehicles (PIVs) at the facility. PIVs used at the facility include, but are not limited to; the company's propane sit-down fork trucks [Mitsubishi Model #: FGC30N S/N: AF83F40232 and Mitsubishi Model #: FGC25N S/N: AF82F44423].
