29 CFR 1910.140(c)(22): The employer did not ensure that personal fall protection systems are worn with the attachment point of the body harness located in the center of the employee's back near shoulder level. The attachment point may be located in the pre-sternal position if the free fall distance is limited to 2 feet (0.6 m) or less: On or about 7/6/2022 at the jobsite located at 275 Giralda Ave, CORAL GABLES, FL 33134, an employee was exposed to inversion from a fall due to attachment point of the Weanas Thicken Climbing Harness, Protect Waist Safety Harness, Wider Half Body Harness (Model Z-Y) not being attached to the dorsal position of the harness while utilizing a Sky Genie shock absorbing lanyard (Model #LS-2S-48, Serial #307483) with a maximum fall distance of 4 feet.
