29 CFR 1910.140(c)(18): The employer did not ensure that personal fall protection systems are inspected before initial use during each workshift for mildew, wear, damage, and other deterioration, and defective components are removed from service. a. On or about September 21, 2023, at Peters 1-6, a personal fall protection harness had a rust at a metal latch, frayed fabric edging, and was used by an employee to climb the derrick ladder which exposed the employee to fall hazards. b. On or about September 21, 2023, at Peters 1-6, ladder climb assist hooks did not require 2 separate consecutive movements to open the hooks, and a hook had a broken latch. The climb assist hooks were used by an employee to attach to a personal fall protection harness to climb the derrick ladder which exposed the employee to fall hazards.
