29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: a) On or about December 23, 2013, at the above addressed job site, employees were exposed to 110VAC shock and electrocution hazards while working with a roll forming machine in the sheet metal department which lacked a cover on a junction box that contained live conductors.
