29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control: On or about March 8, 2018, employees were exposed to struck-by hazards when the machine specific LOTO procedure for the Viking Bagger #6 said the following: Verify by pushing the START STOP switch. However, there were no button or switch labels on bagger #6. Employees prepare the machine to run at the start of each shift. Employees change film and clear jams during the shift. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
