29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about February 21, 2019, and at times prior thereto, employees were conducting aquatic maintenance and repair activities, to include diving operations, without the employer providing training to employees in the recognition and avoidance of hazards, exposing employees to the hazards associated with maintenance work performed on, near and under water.
