29 C.F.R. � 1926.1203(d): When the employer decided that employees it directs would enter a permit space, that employer did not have a written permit space program that complies with 1926.1204 implemented at the construction site: On or about 4/24/2019, at 20351 Sheridan Street, Pembroke Pines, Florida, the employer allowed employees to conduct work in a catch basin that had a hole sealed with a Styrofoam insert / hole former, and did not develop and implement a permit space program, thus exposing employees to being engulfed by mud and water.
