29 CFR 1926.451(f)(4): Part(s) of a scaffold that were damaged or weakened, such that the scaffold's strength was less than that required by paragraph (a) of this section, were not immediately repaired, replaced, braced to meet those provisions, nor removed from service until repaired. On or about February 13, 2020, at the New Sherman High School Project, visible defects in scaffolds were not immediately corrected by replacement or repair prior to allowing employees to access unsafe scaffold systems exposing employees performing masonry activities to fall hazards of up to 18 feet to the concrete below.
