29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: (a)Forest Products Distributors, Inc. at 5406 Old Folsom Road, Rapid City, SD 57703: On or about and at times prior to May 12, 2022, the employer did not ensure that forklifts with defects rendering the forklift unsafe to operate, such as non-functional seatbelts or horns were removed from service. This condition exposed employees to struck-by hazards.
