1910.147(d)(3) Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). 29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6): On or about November 2, 2022, the employer did not ensure that all energy isolating devices that were needed to control the energy of Slitter 74 were physically located and operated in such as manner as to isolate Slitter 74 from its energy sources. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
