29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. a) On or about October 19, 2018, or prior thereto: The Caterpillar powered industrial truck, model# GP25, S/N #SAM91562, was modified with a front attachment to lift wood onion bins in the facility. The onion bins weigh up to 1,750 pounds when full. The modification was made without identification of the weight of the attachment and powered industrial truck, and determining a modified load capacity, prior to conducting lifts at the facility. NOTE: Abatement certification is required for this item.
