29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: a. On or about August 14, 2019, at the above addressed jobsite, an employee was using and riding on a job made work platform attached to the forks of a rough terrain powered industrial vehicle that was not secured to forks, thereby exposing the employee to fall hazards of approximately twenty-three (23) feet.
