29 CFR 1910.178(l)(3)(ii)(A):Powered industrial truck operators did not receive initial training in surface conditions where the vehicle would be operated that could affect safe operation: a. On or about November 25, 2020, at the above addressed jobsite, powered industrial vehicle (i.e., forklift) operators were not provided with initial training related to the surface conditions where the vehicle was operated, thereby exposing employees to vehicle tip-over and crushed-by hazards.
