29 CFR 1910.147(d)(4)(i):Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: Maintenance and laser operator did not affix energy control devices to Trumpf laser material handling lifts: a. On or about September 17, 2021, a maintenance employee entered the material handling area to adjust slat bolts on index table. b. On or about September 17, 2021, and October 19, 2021, laser operators entered the material handling area to clean the index table slats. c. On or about October 01, 2021, to October 08, 2021, laser operators entered the material handling area to replace index table slats. d. On or about September 17, 2021, laser operators entered the material handling area to perform cleaning activities.
