29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained. On or about November 2, 2022, the employer did not ensure that trained authorized employees participated in group lockout per 29 CFR 1910.147(f)(3)(ii)(D). The employer did not ensure that operators affixed a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when performing service and maintenance tasks on Slitter 74, such as monthly PMs, with maintenance personnel. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
