29 CFR 1910.212(a)(2):General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. a. On or about February 7, 2023, employees operating SAM 12 machine were exposed to the point of operation. Plastic guarding was not securely affixed to the exterior, which exposed employees to caught-in hazards. b. On or about February 7, 2023, employees operating SAM 20 machine were exposed to the point of operation. Plastic guarding was not securely affixed to the exterior, which exposed employees to caught-in hazards. c. On or about February 7, 2023, employees operating SAM 36 machine were exposed to the point of operation. Plastic guarding was not securely affixed to the exterior, which exposed employees to caught-in hazards. d. On or about March 14, 2023, employees operating SAM 11 machine were exposed to the point of operation. Plastic guarding was not securely affixed to the exterior, which exposed employees to caught-in hazards. e. On or about March 14, 2023, employees operating SAM 19 machine were exposed to the point of operation.
