29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: a) Production Area: A pair of keyed shaft ends protruding into an aisle way near the outside corner of the International box folding machine exposed workers to an entanglement hazard, on or about September 8, 2014. b) Production Area: The unguarded speed adjustment wheel on the International box folding machine projected more than half the diameter of the shaft end exposing workers to an entanglement hazard, on or about September 8, 2014.
