29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. a. Tiffin Metal Products Co. located in Tiffin, Ohio: On or about March 12, 2015, the employer did not ensure an employee performing die set up on the Garco 65 ton full revolution mechanical power press located in the Building #2 welding department was applying a lockout device to the electrical disconnect, dissipating pneumatic energy from the clutch mechanism, and inserting die blocks prior to performing set-up operations. b. Tiffin Metal Products Co. Located in Tiffin, Ohio: On or about March 19, 2015, the employer did not ensure an employee performing set up on the Piranha ironworker located in the Building #1 "Pit" was applying a lockout device to the electrical disconnect prior to performing set-up operations on the equipment.
