29 CFR 1910.333(a)(1): Live parts to which an employee was exposed were not deenergized before the employee worked on or near them: A) Humanetics Innovative Solutions, Inc., located at 900 Denton Drive, Huron, OH 44839: On or about December 26, 2014, the employer did not ensure that an electrical panel, located in the maintenance room, was disconnected from all electric energy sources when a maintenance employee was wiring the battery charger. The employee was exposed to 480 volts/3 phase electrical hazards. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
