29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) Throughout facility: The employer did not ensure that employees operating powered industrial trucks, such as, but not limited to, a Yale forklift, model ERC050ZFN48SE082, serial number E108V09883V, Raymond reach lift, model 20.R30TT serial number 020.88.27629, Raymond reach lift, model 20.R30TT serial number 020D-90-32355, Yale electric pallet jack model MPE040LBN24S2748ee serial number N539654, Lift Rite Ergo electric pallet jack model EA 12/10L, and Crown electric pallet jack model 25WBTL-S serial number H12198, received training and an evaluation as specified by the standard, on or about 7/13/15.
