29 CFR 1910.179(d)(4)(iii): Fixed ladder(s) on crane(s) were not permanently and securely fastened in place: a) North dock crane: On or about March 4, 2016, and at times prior, workers were exposed to falling more than 60 feet to ground level when using a ladder on the crane bridge while the right ladder siderail was not attached to the bridge deck and the bottom rung was detached from the left ladder rail.
