29 CFR 1910.303(b)(1)(iv): Electrical equipment was not free from recognized hazards, based on the electrical insulation; On or about June 22, 2016, E.R.H. Enterprises, Inc. had employees exposed to electrical hazards from exposed electrical wiring due to missing outer electrical insulation on flexible cords providing power to equipment at the water treatment plant in Danville, Illinois. These instances included: a. Flexible electrical cord coming out of electrical panel on the north side of DAF 3 and b. Flexible cord at the connection point of water pump providing electrical power to reserve water tanks.
