29 CFR 1910.332(b)(2): Employees who are covered by paragraph (a) of this section but who are not qualified persons were not trained in and familiar with any electrically related safety practices not specifically addressed by 1910.331 through 1910.335 but which are necessary for their safety. Pier 3, Crane 2, Machine House: Prior to March 10, 2017, crane heavy equipment mechanics who maintained the Transformer Secondary Substation System (TSS) equipment removed filters and their frames which potentially exposed them to live 4160 volt bus bars inside the TSS that had not been locked out or tagged out. On March 10, 2017, a crane heavy equipment mechanic removed a rear panel cover of the Transformer Secondary Substation System (TSS) equipment near two energized 4160 volt mechanical bus bars. The TSS was not de-energized prior to the work. The mechanic contacted the bus bars and was electrocuted.
