29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: The employer is failing to provide employees with ladder safety training prior to allowing them to utilizing ladders on site. This training would allow each employee to recognize the hazards of improperly using ladders in order to minimize these hazards. The workers were exposed to falls of over 14 feet to dirt. This was most recently documented at the corner of 204th St. and Maple St., Elkhorn, Nebraska on or about June 21th, 2017 when the employer had workers accessing the roof of the building being constructed on site by stepping on the top two steps of the ladder.
