29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided. a). At the worksite - On or about June 19, 2017, a stairway or ladder was not provided to an employee for access to the pivot point of a Valley Pivot Point Irrigation System (model: 7000/8000) to perform alignment of the pivot point and the first tower flanges, thus exposing the employee to a fall hazard of approximately eight feet.
