Text For Citation: 01 Item/Group: 004 Hazard:
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employer failed to train employees in recognizing and avoiding caught-in hazards associated with moving parts of machines and in proper use of guards: a) Bay 8- On July 19, 2017, the employer failed to ensure employees were trained to guard hazards created by the moving saw blade on either side of the pint of operation on the Amada Horizontal Bandsaws S10A and S26A by correctly using the movable guard. b) Bay 8- On July 27, 2017, the employer failed to ensure that employees were trained to guard hazards created by the moving Horizontal Bandsaws S9A, S22A, and S23A by correctly using the movable guard. c) Bay 8 - On July 19, 2017, the employer failed to ensure that employees were trained on safe work practices concerning hazards created by the point of operation on the Tsune 20 Circular Sawing Machine. d) Bay 8 - On or about July 13, 2017, the employer failed to ensure that employees were trained on safe work practices concerning hazards created by the point of operation on the Tsune 21 Circular Sawing Machines. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
