29 CFR 1926.417(a): Controls that were to be deactivated during the course of work on energized or de-energized equipment or circuits were not tagged: Rover Petroleum, the Hoose Battery, Hoose #4 Ira, TX: On or about 22 September 2017, employees were exposed to electrical shock hazards while installing a new connection between an old power box and a new one on a commercial power distribution pole that had not been de-energized or tagged.
