29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): (a) On or about March 26, 2019 - 660 Industrial Blvd., Albertville, AL, employees were exposed to struck-by hazards when there was no policy to take out of service powered industrial truck(s) that had a nonworking backup camera, that required the addition of hydraulic fluid/engine oil/coolant, that leaked transmission fluid/hydraulic fluid, that had a nonworking backup alarm, and/or that had a nonworking side shifter.
