29 CFR 1926.1204(c)(3): The employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations by isolating the permit space and physical hazard(s) within the space: On or about 04/24/2019, at 20351 Sheridan Street, Pembroke Pines, Florida, the employer failed to remove a Styrofoam insert / hole former from a catch basin, and insert a plywood board or metal plate over an opening to protect the space from mud and water entering the catch basin, while an employee was working inside, thus exposing the worker to an engulfment hazard.
