29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled. The employer is failing to prevent employee exposure to struck-by / crushing hazards. At 6190 Mid Rivers Mall Drive in St. Peters, MO 63304 - On or about 07/23/2019, employees were exposed to the hazard of being struck by elevated loads where a "Crown" (Model No. RR5725-45; Serial No. 1A422917) powered industrial truck (PIT) was loaded with about 26 sheets of 5/8-inch drywall, measuring 48 inches in width by 144 inches in length, having a combined weight of approximately 2,745.6 pounds, was unstable and unsafely arranged on 24-inch centered PIT forks.
