On or before August 8, 2019, where employees worked in house 7, the lower belt guard of row 1 was missing, thereby exposing employees to caught-in hazards. b. On or before August 8, 2019, where employees worked in house 7, the lower belt guard of row 2 was loose and missing screws, thereby exposing employees to caught-in hazards. c. On or before August 8, 2019, where employees worked in house 7, the lower belt guard of row 4 was loose and missing screws, thereby exposing employees to caught-in hazards. d.
