Amtrak - Hellgate substation 46: An employee was using an extension ladder that was not secured to access fuses on a high voltage substation transformer approximately 12 feet from the ground. On or about, December 5th, 2020. NOTE: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
