29 CFR 1926.1204(e)(1)(i): The employer did not perform pre-entry testing of the permit space before entry was authorized: On or about July 9, 2020, at or near the intersection of Mitchell St. and W. Barkley St. in Sour Lake, TX, where two employees were exposed to toxic sewer gases and engulfment hazards after entering a sewer manhole without the confined space atmosphere being tested before entry.
