29 CFR 1926.1431(g)(1)(iii): Detachable devices for personnel platforms were not closed and locked when attached. a) On or about March 21, 2021, at the jobsite, a personnel platform was used to hoist employees to install the arms on a power line monopole without the following: 1. Locking the bottom pin connecting the jib adaptor to the jib nose. 2. Locking the two pins connecting the platform yoke to the jib attachment. 3. Using the retaining bolt to secure the platform assembly to the jib assembly.
