29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: This violation was observed on or about November 3, 2021, at the facility when employees conduct servicing and maintenance on equipment without using lockout/tagout devices, exposing employees to caught-in hazards. Pursuant to 29 CFR 1903.19, within (10) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure the use of lockout/tagout devices while servicing and conducting maintenance on equipment.
