29 CFR 1926.652(a)(1): Each employee in an excavation is not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. a) On April 21, 2022; the employer did not protect employees working to install two 6" conduits in a trench without cave-in protection at a depth ranging from 4.0-6.0 feet with nearly vertical walls and Type B soil. This work was occurring directly adjacent and parallel to a previously installed utility line with unstable backfill around it. Note - Abatement certification and documentation are required for this item.
