29 CFR 1910.334(a)(2)(ii): When there was a defect or evidence of damage that could expose an employee to injury, the defective or damaged item was not removed from service until the repairs and tests necessary to render the electric equipment safe had been made: a) On or about December 23, 2013, at the above addressed job site, employees were exposed to shock and electrocution hazards while working with energized 110 VAC electrical cords that had damaged outer insulation, and a plug that lacked strain relief, cords that had damaged outer insulation, that powered floor fans and hand tools and had not been removed from service. 3 instances: 1. A box fan in the shipping department had damaged outer jacket insulation that had been taped. 2. An extension cord in the shipping department had damaged outer jacket insulation that had been taped. 3. An extension cord in the shipping department had a plug that lacked strain relief.
