29 CFR 1910.147(e)(1): Before lock out or tag out devices were removed, the work area was not inspected to ensure that nonessential items had been removed or ensured that machine or equipment components were operationally intact: a) On or about February 9, 2019, before lockout devices were removed from the upper column of DU2 (after a turnaround and before startup activities had begun) the work was not inspected to ensure equipment or piping were operationally intact. An 18 inch line from an 1st Stage Separate was left with a valve open and a spool piece removed from the tube side of vent exchanger T4330.
