29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service: a) On or about February 21, 2024, in the pump equipment area, where extension cord used to power the Drummond 1/3 HP submersible utility pump had damaged insulation, exposing employees to electrical shock and burns hazards.
