29 CFR 1910.119(e)(1): The process hazard analysis was not appropriate to the complexity of the process and did not identify, evaluate, and address the control of the hazards involved in the process. a)This condition was observed at the E. I. du Pont de Nemours and Company, in Beaumont, TX, where the employer did not ensure that the Process Hazard Analysis (PHA) was appropriate to the complexity of the process, and that the PHA identified, evaluated and addressed control of the hazard involved in the process when the employer failed to ensure that RV-J-0111 relieved to a safe location, therefore exposing employees to an atmosheric release of toxic material.c
