29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): At the site on or about March 21, 2016; employees operated equipment that was not guarded to prevent them from having any part of their body in the danger zone, exposing the employees to amputation hazards. a) In the loading bay an employee was exposed to seven inches of unguarded blade while operating a Lexas horizontal band saw, model #: LX-330NC. b) In the loading bay an employee was exposed to 6 inches of unguarded blade while operating a DOAL horizontal band saw, unknown model and serial number.
