Bakery: The conveyor belt located next to the Nwik Lok bag closing machine, Type # 1082J8, Model MR, Serial # 31397 was missing a piece of guard that enclose the shaft and the ingoing nip point of the roll and conveyor belt, observed on or about 05/04/2016.
