29 CFR 1910.304(f)(2)(ii): The employer did not ensure that each protective device shall be capable of detecting and interrupting all values of current that can occur at its location in excess of its trip setting or melting point; (a) Computer Main Panel : Breaker on electric panel was locked in closed position by a screw placed to hold it. Conditions prevent the breaker from tripping to open in case of overcurrent creating fire and other electrical hazards, on or about December 22, 2016.
